Dental Information

We are committed to treating you with the compassion and respect you deserve. We believe in providing skilled dental services that address both your short and long-term dental care needs in an honest, judgment-free environment where it all comes down to great care.

Dental Services

 Dental services include exams, x-rays, dental and health care instructions, cleanings, fillings, extractions, full dentures, partial dentures.

If you are experiencing an URGENT DENTAL PROBLEM, you may come to the Dental Emergency Clinic. A dental emergency is defined as dental pain, discomfort, swelling, and signs/symptoms of infection. You must be present at 8:15am Monday, Wednesday or Thursday with all necessary documents.

This is a lottery drawing not "first-come, first-serve" program based on dentist availability. There is no guarantee you will be seen the day you come.

Basic Qualifications

  • Be 18 years old or older

  • Be a resident in one of the following counties:

  • Indiana - Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wabash, Wells, Whitley;

  • Ohio - Defiance, Paulding, Van Wert, Williams

Required Documentation

  1. Photo identification (drivers license, employee ID, etc.).

  2. Proof of residence

  3. Proof of all household income for one month

  4. Proof that you have applied aid

  5. The names, dosages, and frequency of all medications you are taking.

The Eligibility Specialist will inform you when the above documentation is due for registration as applications cannot be processed without them. You may not see a provider the same day that you register, but you will be given the first available appointment.

Costs

There is no fee charged to you for services at Matthew 25. However, to help defray the costs of the clinic, patients are asked if they would like to give a free-will donation of $10 for dental visits.

There are fees for dental appliances such as dentures, partials, 

flippers, and nightguards. Prices are discussed with patients after an appointment as costs vary with size and materials used.

Any tests or procedures performed at Matthew 25 are done without charge. If it needs to be done elsewhere, then special arrangements are made through Matthew 25 by giving you a “referral slip”, but you still may be responsible for some of the cost.
