Dentist (DDS)
Duties
Core privileges: Include the examination and treatment of patients as shall normally be considered part of general dentistry. Such privileges include the ordering and interpretation of diagnostic imaging, diagnosis of oral diseases and conditions and treatment planning. Treatment provided may include local anesthesia, exodontia, tooth restoration, palliative relief from severe pain, fabrication and delivery of prostheses, and the ordering of medication.
Special requests: In order to be granted privileges for additional procedures, the applicant may be required to provide evidence certifying competence.
Additional privileges requested:
Qualifications
Basic education: DDS or DMD
Minimal Formal Training and Certification:
Graduation from an accredited dental school
License to practice dentistry in the state of Indiana
Current CSR registration
Current CPR (BLS) certification
Minimum Age
21
